New Zealand planned a stamp issue in 1996 on the theme “Children's Health - Safety on the Road". When the stamps came off the printer, it was noticed that the 40c stamp had a design flaw. The teddy bear in the image was strapped into an adult seat belt, so it was effectively facing the direction of travel. This meant that the baby sitting next to the teddy in the baby seat was also facing the direction of travel. However, it is regulation to fasten the baby seat against the direction of travel. For this reason, stamps were printed without the teddy bear before the issue date, so that the impression of the incorrect orientation did not occur.

The stamps were then issued on June 5, 1996. Printing was done wet-adhesive in sheets of 100, as souvenir sheet and the 40c issue also for the first time in self-adhesive form. In at least 2 post shops, however, copies of the stamp with faulty design were sold, both wet-adhesive and self-adhesive.
